In order to update your name we require a copy of certification (e.g. marriage certificate or deed poll). As such, we ask that patients fill in a ‘Change of Name’ form in practice with an appropriate certificate to hand.

To update your current address, please get in touch to fill in a form in practice. We will also ask for a proof of address – this can be a utility bill, bank statement, government documents/correspondence, form of ID, etc.
